HERMANN could give the keynote speech and talk about latest trends in Periodontics, Implant and Esthetic Dentistry at the University of Sharjah / Dubai (United Arab Emirates [UAE]), the first and largest Dental Hospital in the UAE and the overall Gulf Region as invited speaker of the 3rd Sharjah Dental College & 16th Emirates Medical Association International Conference. The University Dental Hospital Sharjah (UDHS) / Faculty of Dentistry was founded in 2004 and is integrated in the compound of medical and health sciences. Therefore, a comprehensive approach in Medical Sciences was of major importance during this international conference focusing on the Art of Dentistry as an important branch of Medical Sciences and not on a separate discipline by itself.

Chronic (type II periodontal disease) and aggressive periodontitis / periimplantitis (type III periodontal disease) occur in about 80 % (60% / 20 %) of all patients in our today’s clinical practices. Thus it has become of utmost importance to precisely identify and distinguish type II vs. type III periodontal diseases to be able to primarily and completely cure such different illnesses (inflammation vs. infection) and secondarily maintain the state of health based upon an individual bio-logical risk assessment and a customized maintenance care protocol long-term. In that regard, Prof. Joe HERMANN could talk at the ‘Annual Berlin Prevention Days’ to about 350 Dental Hygienists, Prophylaxis Assistants and their General Dentists upon invitation by the Philipp-Pfaff-Institute in Berlin (Germany). As one of the major conclusions, it has become clear that such an approach will provide the least incidence of dental urgencies in the daily dental practice, and especially, will also allow to even more focus on a sophisticated level Dentistry with increased financial benefits both for the patient as well as the team of Dental Professionals.

The German Association of Oral Implantology (DGI) celebrated ‘60 Years of Periimplant Hard Tissue Integration’ with 3’500 participants at the occasion of the 25th DGI Annual Meeting taking place in Dresden (Germany). As a keynote speaker, Prof. Joachim S. HERMANN could present his latest news on a bio-logically driven implant placement protocol, and in particular, on eight-year results of successful immediate implant loading (100% Success Rate) in single and multiple tooth gaps in partially edentulous patients which may provide the least / no amounts of buccal and crestal Bone Loss (cBL) as well as minimal / no amounts of recession of the Gingival Margin (GM) long-term.

For the fifth time, the ZP® Dental Excellence™ Study Club joined in at the DSI® Headquarters in Zurich (Switzerland) comprised of referring general dentists as well as specialists and their teams discussing a bio-logical approach to come up with predictable and reliable implant esthetics long-term. As a crucial aspect of this DSI® Continuing Education event, it has become clear that a different biology seems to be involved with Bone Level Implants as opposed to Soft Tissue Level Implants as well as the fact that switched platforms are supposed to react differently in comparison with conventional matched platform implant designs. As a conclusion, it has been suggested - at the basis of both experimental and clinical data - that the best results can be achieved when using a Soft Tissue Level approach in combination with a matched platform implant design.

Prof. Joe HERMANN was invited to Basel (Switzerland) by the Swiss Association of Dental Laboratories (VZLS) in close collaboration with the Institute Straumann AG, presenting on periimplant crestal hard and soft tissue biology both from a clinical as well as experimental point of view comparing Bone Level vs. Soft Tissue Level Implant approaches. As a major conclusion, it became evident that different locations of the crestal Bone Level (cBL) will directly influence the level of the Gingival Margin (GM) according to the principle of the Biologic Width (BW). Thus, perfect periimplant ‘Red’ as well as ‘White Esthetics’ are directly dependent on each other and related to a bio-logically-driven ‘Tissue-directed Implant Placement’ as well as a precise ‘Restoration-driven Implant Placement’ Protocol (3-D-Implant Positioning).

Upon invitation by the Upper Lake Zurich Rotary Club from Rapperswil (Switzerland), Prof. Joe HERMANN could present on the interaction of aggressive periodontitis and heart attack as well as stroke and multiple other severe general diseases / conditions. In that regard, it could be discussed that such an aggressive oral disease occurs at a rather high percentage of about 20% of the overall world-wide population. Furthermore, it has become clear that aggressive periodontitis / periimplantitis does not just trigger such potentially fatal diseases, but really can directly be responsible for such diseases. Thus, a completely healthy oral cavity can be a major contributor for patients’ general health and longevity in general.

The first DSI® Expert Level Academy Meeting took place at the national French DSI DentaScience Institute® in Grenoble (France). Major focus was given to the anatomy, handling and preservation of Periimplant Soft Tissues. As an integral part of this meeting format, about 50% of the overall duration of this high-end course was a Hands-On seminar, allowing each participant to directly transfer unique Biological Principles into daily clinical practice (Program / Testimonials / Photo Gallery). Early 2012, the appropriate Periimplant Hard Tissue Course will take place in a modular fashion and as an ideal combination. Furthermore, the Periimplant Soft Tissue Course will be repeated during the fall season of 2012 as well.

During the Eurosymposium 2011 in Constance (Germany), Prof. Joachim S. HERMANN spoke on Oemus Media TV / DSI® YouTube Channel about the State-of-the-art in Periodontics and Implant Dentistry in 2012. Nowadays, there is ample evidence for the fact that Dental Implants should only be placed in a completely healthy oral cavity when trying to achieve complication- and / or failure-free as well as esthetic long-term results. This is even more important since about 70% (!) of the overall world-wide population suffers from periodontitis. Therefore, it seems to be crucial to transfer unique Biological Principles into daily clinical practice. Furthermore, a healthy status of both teeth and implants should be monitored by a specialist team dealing with a meticulous analysis of the medical history, diagnostic work-up, active therapy as well as maintenance care long-term.

At the occasion of the 6th South German Implantology Days in Constance (Germany), Prof. Joe HERMANN could present on latest trends as well as challenges and limits of Dental and Oro-facial Diagnostic Sciences. A major aspect was the fact, that every single diagnostic tool - from periapical radiography up to Cone-beam Computer Tomography (CBCT) - should be used where necessary and based upon a specific indication (Oral Surgery / Periodontics / Endodontics / Orthodontics / Implant Dentistry / Prosthodontics). As of today, the CBCT technology has become the ’Standard of Care’ in multiple compelling diagnostic indications in analogy to the introduction of conventional panoramic radiography (Pano) some 40+ years ago. As a consequence, oro-facial treatments can be carried out significantly more precisely and in a (more) minimally-invasive fashion. Another interesting fact is the significant reduction in radiation doses of about 75% comparing CBCT with conventional head and neck Computer Tomogram (CT) technology (Periapical Radiograph ca. 1x : Pano 4x : CBCT 16x : CT 64x). Furthermore, it seems to be important to understand that Board Certified Radiologists (Germany) need to carry out and analyze 2’500+ imaging examinations while going through a six-year full-time residency. This explains the fact that Dentists as well as Physicians using CBCT technology do have a great responsibility and should have a record of extensive continuing education, training and experience when using this unique technology.

Under the guidance of Prof. Dr. Joachim S. HERMANN (’Principal Investigator’), an international research team (United States of America / Switzerland / Germany / Jordan) could show for the first time different both periimplant hard- and soft tissue reactions in relation to the implant insertion depth as well as implant surface characteristics comparing a relatively smooth, machined titanium surface with a Sandblasted, Large-grit and Acid-etched Surface [SLA]). In this first peer-reviewed publication (ISI® Impact Factor: 2.479 / Ranking: # 9) it could be shown that the amount of crestal bone loss could be decreased significantly. Furthermore, periimplant connective tissue fibers adjacent to SLA surface textures showed a perpendicular fiber orientation into the surface as opposed to scar-tissue like connective tissue characteristics in relation to relatively smooth machined titanium surfaces.

In recent years, major international epidemiological studies have clearly shown that only about 5% of our world-wide population are periodontally / periimplantologically healthy, ca. 10% exhibit a periodontal / periimplant gingivitis (type I A periodontal disease - low-risk patient group), about 65% of all patients do have a chronic periodontitis / periimplantitis (type II periodontal disease - low-risk patient group), whereas ca. 20% (!) suffer from aggressive periodontitis / periimplantitis (type III periodontal disease - high-risk patient group). A challenge in today’s successful clinical practices remains the ideal assignment of Prophylaxis Assistants, Dental Hygienists, General Dentists, and Periodontists to the appropriate patient groups based upon the specific difficulty level (Course Objectives /Summary).

Two weeks after the first ZP ZurichPeriodontics® Dental Excellence™ Study Club meeting, further referring general dentists and dental hygienists from several countries joined in at the DSI DentaScience Institute® in Zurich focusing on ’Distinguishing Criteria for the Detection of Chronic vs. Aggressive Periodontitis / Periimplantitis ?! Can a complete oral healing be achieved and maintained long-term ?!’ (ZPSC02/2011). One of the major issues of this seminar was the identification of 12 specific criteria allowing to distinguish a chronic periodontitis / periimplantitis (low-risk patient group) from an aggressive periodontitis / periimplantitis (high-risk patient group).

The first ‘International Symposium on Cone-Beam Computer Tomography (CBCT)’ took place in Warsaw / Poland, where Prof. Joe HERMANN could present on long-term results related to the topic “How much Dental Diagnostic Sciences Do We Need ?! - From Periapical Radiography to Cone-Beam Computer Tomography (CBCT)”. It became clear as a conclusion of this international conference that up-to-date Radiology / Dental Diagnostic Sciences are not just separate, descriptive sciences any more, but have become clinical sciences and thus, a crucial part of a sophisticated daily clinical practice. In addition, more and more synergies are being developed with specialists in the field of Ear, Nose and Throat (ENT) Medicine, which will allow improve both the quality of ENT as well as Dental Medicine.

Prof. Dr. Joachim S. HERMANN was one of the main speakers at a Hamburg (Germany) Symposium on the relationship between Periodontitis and General Health Issues. As of today, there is ample evidence that aggressive periodontitis / periimplantitis (type III periodontal disease) can directly cause or exaggerate the four most lethal diseases world-wide (World Health Organization [WHO]) like heart attack, stroke, pneumonia, and chronic obstructive pulmonary disease (COPD). Also endocarditis, rheumatoid arthritis, premature delivery, pre-term low birth weight, diabetes mellitus, osteoporosis, malignant tumors in the oral & maxillofacial area and rejection of breast implants seem to be linked to a transferable infectious type III periodontal disease (‘Kissing Disease’) why a completely healed and healthy oral cavity (!) seems to be of utmost importance.

The Straumann® company launches a new membrane for Guided Bone Regeneration called MembraGel™ world-wide and introduces it to the Swiss market. The membrane is comprised of polyethylene glycol (PEG) which is applied as a liquid and sets in situ. As this is an innovative technology requiring different handling to conventional membranes, Straumann® is combining the launch with an education program that includes hands-on product training, making the product available only to dental professionals who have participated in a particular training program.

HERMANN (right) initiate a close collaboration between the DSI® and the SSC-Hamburg at the occasion of Dr. SOLAKOGLU’s visit of the DSI® International Headquarters in Zurich. Major focus of all joint continuing education (CE) events in the future will be the field of Periodontics and Implant Dentistry, based upon an intense collaboration of Dental Hygienists / General Dentists and Specialists.
